FAQ'S For Medi-Cal & Healthy Families Members

Whom should I call if my doctor’s office says I no longer have Medi-Cal?  You should verify with the health plan; Alameda Alliance for Health or Anthem Blue Cross.  Then call Alameda County Social Services at 510-639-1007 or 800-698-1118, or contact your social worker.

What should I do when I receive a bill from a billing company?    Call the billing company and make sure they did not send the bill in error.  If they do not have your health plan information, give them your health plan I.D number (found on the front of your ID card) and not your social security number.  You can also call CFMG Provider Relations for assistance at 510-428-3154.

Whom should I call for Dental, Vision services, or Mental Health services?

Alameda Alliance for Health

•    If you participate with Alameda Alliance for Health Medi-Cal, your dental care is covered by Denti-Cal.  You may contact Denti-Cal directly at 800-322-6384. Vision care is provided by March Vision Care and may be contacted at 888-493-4070.
•    If you participate with Alameda Alliance for Health Healthy Families, Healthy Families Dental Plan, 888-747-1222, covers your dental care.  Vision care is provided by Healthy Families and may be contacted at 888-747-1222.
•    If you are an Alameda Alliance for Health Medi-Cal member, call March Vision Care, 888-493-4070. For Dental, call Denti-Cal at 800-322-8384. For Behavioral Health Medi-Cal patients, call Alameda County Behavioral Health at 510-567-8100. For Healthy Families call PacifiCare Behavioral Health, Inc. at 888-789-7110.

Anthem Blue Cross

•    If you participate with Anthem Blue Cross,  your vision care is covered by Vision Service Plan (VSP). You may contact VSP by calling  800-877-7195. For Dental care, call 800-880-5305. For Behavioral Health, call Alameda County Mental Health Services at 510-567-8100. For Healthy Families, call 800-399-2421. For Contra Costa Mental Health Services at 925-957-5150.
